Collaborates with system healthcare professionals to ensure safe and cost-effective utilization of medications. Supports activities that impact the health-system formulary (e.g.. monograph development, class reviews, therapeutic interchanges and non-formulary requests, REMS program requirements, maintenance of online-formulary, staff communications, website updates). Serves as system coordinator for drug shortages and response management. Serves as system coordinator for formulary implementation/change activities. Collaborates with pharmacy informatics and medication safety teams to promote safe use of medications. Serves as coordinator for Medication Utilization Evaluation activities for the system. Provides oversight for the development and maintenance of medication-related policies and procedures. Responsible for the monthly Pharmacy Newsletter and ongoing communication of key formulary-related activities. Serves as a resource for Drug Information inquiries. Serves as Drug Information preceptor for pharmacy residents and students.Continued personal and professional growth and development. Performs other duties as assigned or requested.MINIMUM QUALIFICATIONS:
Doctor of Pharmacy degree from an accredited School of Pharmacy; required. Advanced Pharmacy Residency training; required (PGY-2 Drug Information, Medication Safety or Medication Policy preferred). Current Georgia Pharmacy License or reciprocation eligibility for licensure in Georgia. BCPS Certification required within 2 years of hire. Minimum of three (3) years' experience; required (Residency training years may count toward these years). 5 years preferred. Experience with coordinating system-level formulary approvals and implementation within integrated databases required. Experience in an academic medical teaching health system or a large community teaching hospital pharmacy environment; preferred.JOIN OUR TEAM TODAY!
