We are seeking a Part Time Early Intervention Service Coordinator to join our team in Bensalem, PA.

 

 

Position Details

Maintains caseload of approximately 25 families with minimum monthly contact. Acts as a liaison with Bucks County MH/DP, Behavioral Health, Children & Youth, referring agencies, families, child advocacy representatives/organizations, and stakeholder service providers. Assists in ensuring program compliance with all internal and State/County policies, regulations, and requirements. Complete intakes according to Merakey of Bucks County policies and procedures. Facilitating and participating in the assessment/evaluation, development, implementation, review of IFSP’s as required by program regulations and family needs. Assess child needs and family concerns. Secure pertinent information regarding child development. Coordinate implementation of intervention services. Educate families on outcomes and the family-training, routines-based model of intervention. Assisting the family in identifying available service providers and facilitating communication with and between the family and service provider.

Earn: $22.89 per hr.
