POSITION SUMMARY: 
MSW Medical Case Management and Early Intervention Services are the provision of a range of client-centered activities focused on improving health outcomes in support of the HIV care continuum. The MSW MCM/EIS Lead 
is a professional who collaborates with physicians, health care team members and patients to coordinate care for specific patient populations across the continuum. Provides various social work services to meet needs of patients 
including leading groups and mental health crisis interventions in addition to routine medical case management activities: biopsychosocial assessment and reassessment, care plan, insurance navigation and linkage to other resources necessary for patient engagement in care. Provides mentorship, guidance and support to other MCM/EIS case managers. Social work practices and services are consistent with applicable accreditation and licensing standards, and reflect the National Association of Social Worker Code of Ethics.

What you will do:

1. Leads monthly support groups including:

• Scheduling
• Planning
• Providing direction and topics during group
• Guiding discussion
• Participating in process meetings 
• Creating and following relevant policy/procedure
• Providing guidance to 2nd lead colleague
• Reports concerns to manager and engages in 
problem solving. 
McClees clinic is funded for multiple support 
groups. Our funding requires that an MSW is the 
lead facilitator. Both MCM and EIS lead are 
necessary due to the number of monthly groups 
held.
2. As Lead, supports and guides MCM or EIS 
colleagues. Collaborates with manager, 
colleagues and Quality Mgt. on policy/process 
improvement. Includes but is not limited to 
support and guidance around grant requirements, 
funded activities, performance measures and 
quality management/improvement activities for 
each program (MCM or EIS).
3. As Lead, provide routine supervision and 
opportunities for process meetings. Includes 
professional supervision to meet licensing 
requirements as well as supervision specific to 
process change.
4. Conducts biopsychosocial assessments and 
reassessments with patients at defined intervals. 
Completes acuity scale guided by assessment. 
5. Comprehensive, individualized /Service Plan 
developed collaboratively with the client within 30 
days of biopsychosocial assessment or referral 
for services.

What you will need:  

1. Masters of Social Work. earned within 6 months of hire, prior employment experience in a 

related area preferred.
2. Upon hire candidate must have a Bachelor’s degree in Health or Human Services from 
accredited College or University. Prior employment experience in a related area 
preferred.
3. Demonstrate ability and skill in the management of patient care planning and continuous 
quality improvement in services delivered.
4. Demonstrated ability to engage in consistent communication with members of the patient 
care team; participation in case conference or other collaborative meetings where 
decisions about patient care are taking place.
5. Well-developed communication skills, both written and oral. Ability to communicate 
effectively with individuals and groups representing diverse perspectives.
6. Ability to work flexible work schedule.
7. Strict adherence to patient confidentiality expectations.
8. Knowledge of applicable accrediting and regulatory standards.
9. Articulates and demonstrates commitment to the philosophy, values, mission and vision 
of MHP
