ED Crisis Behavioral Health Navigator

Lowell General Hospital is building an ED Crisis Team with LICSWs, LMHCs and licensed eligible clinicians who will work collaboratively with RNs, NPs and MDs to provide evaluations and treatment planning for behavioral health patients in the Emergency Department.  The clinician will work onsite in the ED, and shifts are available for 24x7 coverage. 

Job Overview

As a Behavioral Health Navigator (BHN), you will work on an interdisciplinary team of medical professionals to determine the appropriate care of patients who present to the Emergency Department in behavioral health crisis. BHN collaborates with outpatient providers, DMH, DCF, families, schools, etc. to determine the needs of each patient and to ensure that the patients are receiving the best, most responsive care available. BHNs are part of the hospital community and have the opportunity to be on a variety of hospital and community committees focused on the goal of providing and improving community-based care.

Location: Lowell General Hospital - 295 Varnum Avenue - Lowell, MA and Saints Campus - 1 Hospital Drive - Lowell, MA

Applicants must be able to travel between the two

Hours/Shift: Sunday, Thursday, and every other Friday overnight shift from 7pm-7am - No weekends but will be expected to do select holidays

Minimum Qualifications

Masters in Social Work (MSW)

PhD., LICSW, LMHC, or license eligible

3 years of experience in crisis evaluation and interventions with complex psychiatric patients preferred.

Clinical skill in evaluation of children, adults and seniors

Excellent interpersonal, conflict resolution, and communication skills  

Fluency in a second language preferred but not required

Duties and Responsibilities

The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list.  Other duties and responsibilities may be assigned. 

1. Performs comprehensive psychosocial evaluations to assess the needs of behavioral health patients in the Emergency Department. 

Coordinates information with other health professionals and collateral contacts to identify the appropriate level of care and establish an effective plan of action

2. Collaborates with ED team, PCPs and clinical staff on care management options

3. Provides education/consultation as needed to other mental health care team members and to health care consumers

4. Works with inpatient hospitals and step down programs on the acceptance and transfer of patients who need a higher level of care

5. Serves as a liaison with other service providers or community supports for the continuous improvement of services to patients

6. Provides strength based interventions and education for clients to become actively engaged in their care, improve health literacy, and/or change their health behaviors (e.g. diet, medication adherence, compliance with doctor's visits)

7. Collaborates with the ED team and clinical staff to identify high risk medical/social management patients

8. Makes referrals to community agencies when appropriate

9. Identifies potential legal or risk management issues and discuss with their supervisor

10. Acts as a patient advocate
