Job Overview:
Responsible for the development, coordination, promotion, execution and presentation of Preventive Health Programs for Community members with utilization of evidenced-based clinical information. Responsible for coordinating and completing various health screenings and programs for community members with timely and appropriate follow-up. Works collaboratively with MAP physicians, nutritionists and other health educators to develop and execute educational programs for the community.

Master's in Social Work MSW or Master's in Counseling MC. Bachelor's in Psychology with experience in community services consideredPreferred:

Demonstrates an understanding of needs of the community members and presents health programs focusing in on a variety of health issues by using clinically based information and networking with health care providers and outside organizationsPerform ongoing evaluations of the programs, the instructors and participants to ensure the program meets the needs of the community with successful program satisfaction scores that consistently reach the departmental benchmark of 95 percentWorks collaboratively with other health educators and health care providers to incorporate updated preventive health issues in programsActively takes ownership in the development, implementation and presentation of these programs and becomes known as the expert in the community of Preventive Health topicsActively participates in major Community Health Special Events yearly at the request of the hospital leadershipMaintains a flexible schedule with the ability to work evenings and occasional weekends as neededResponsible to ensuring that there are fewer than five cancelations of programs per year due to low attendanceActively demonstrates the leadership role in educating family members in detail by reviewing health information and explaining results to screening participantsConsistently attends Staff Meetings and contributes as a team memberWorks with department team and creatively self-promotes programs to assist in increasing attendanceChampions opportunities for self-directed continuing educational opportunitiesAttend training sessions/classes at the direction of the department head, and achieve certification that might be associated with the trainingWorks with community partners to provide valuable and necessary lectures, support groups, classes and screenings for our local communityInvolvement in the hospital CHNA, CHIP, and SIP community health improvement plans via programming and sitting on hospital committees related to these initiativesBenefits and Perks:
