Up to $2,500 New Hire Sign-on Bonus

Rate of pay:  $27.00 - $30.00 per hour

 

The Early Intervention Service Coordinator II works with and supports family members and other caregivers to enhance the development of infants and toddlers with developmental delays/disabilities through daily routines and learning opportunities. Functions as a member of a multidisciplinary team using a coaching approach.  Performs service coordination, evaluation and assessment, Individual Family Service Plan development and maintenance.

Responsibilities


Utilizes Routines Based Interviewing to develop understanding of each family’s strengths, needs and priorities as basis for writing goals/outcomes.

 

Conducts intakes and participates in the eligibility evaluation/assessment process.

 

Assists families in gaining access to Early Intervention; facilitates, coordinates, and monitors delivery of services and assists family in identifying all available resources and service providers, making referrals as needed.

Participates in the development of goals, planning and assessment of progress in meeting the goals and objectives of the Individual Family Service Plan IFSP.

Coordinates and chairs as appropriate team meetings including IFSP, IFSP reviews and Transition Conference.

Facilitates the development and implementation of transition planning for each child exiting EI in accordance with state and federal guidelines.

Commits to organizational quality initiatives by participating in programs that will assure quality improvements and team processes.

Documents all activities in accordance with requirements necessary for fulfilling, agency policies or Early Intervention regulations.

Obtains consultation from other members of the team in areas outside own expertise in order to fully address the needs of individual children on case by case basis.

Maintains professional knowledge through participation in professional organizations, conferences and in-services.

Provides direct and/or indirect Early Intervention services as qualified.

Performs evaluations and/or assessments in their specific discipline.

Mentors Service Coordinator I(s) and interns.

Provides formal teaching to parents and other members of the team in area of expertise.



Qualifications

Master’s Degree and/or certification/license in Early Childhood Education or Special Education, Child Development, Social Work, Nursing, Psychology, Nutrition, or related Early Intervention field. 2-5 years’ experience working with infants/toddlers and their families in a family focused, home-based setting; experience in trans-disciplinary provision of support.  Must have valid driver’s license, safe driving record, automobile insurance and access to a vehicle to be used as directed and when needed for transportation. Must have and maintain pediatric CPR certification.
