Skills are measured by:  Observation and verbal feedback from patients, management, directors, coordinator, supervisors, charge nurse, other staff and physicians.  Review of documentation/medical record, time and attendance reporting, in- service records.  Variances from acceptable standards of nursing practice and patient out-comes.
