Position Overview
We have a position available for an Emergency Department Patient Navigator. The Patient Navigator assists unassigned patients presenting to the hospital Emergency Department with selection of a Primary Care Provider (PCP) with one of the hospital's affiliated clinics.
Responsibilities
Utilize WOW to screen patients at bedside in the ED and IP settingsPatient visits will be tracked daily on a spreadsheet and reviewed for possible scheduling of follow up appointment during their normal business dayThe following day, phone communication is made to all patients seen through the Emergency Department during off hours who meet scheduling criteriaSchedule a follow up appointment via Athena for all patients meeting scheduling criteria and provide the patient with collateral for their appointment to include a brochure with the address, phone number and location map for the physician office with the date and time of the appointmentMake a reminder call to the patient the day prior to the scheduled physician office appointmentContact the patient one day post the physician office visit to ensure their satisfaction with the visit, confirm that their needs were met and if there are any concerns needing to be addressed concerning this visit, or future visitsAttend the daily Case Management meetings to review progress on patients who’ve been admitted into the hospital on either outpatient or inpatient basis, as well to determine what the patients discharge status will be for that dayIf the patient is schedule for follow-up exams or tests per the ED physician, the tests will be scheduled in the hospital and the patient Navigator will contact Central Scheduling to provide the exam date/timeIn the event further testing has been ordered, the Patient Navigator will coordinate scheduling at our facility through our Central Scheduling DepartmentMaintain a log of all scheduled procedures and will report daily to the PAS, Physician Practice and CFO/CEOComplete other tasks to include DCA for patients without PCPs, Peripheral Imaging findings/follow up scheduling, FSED and other transfers, follow-ups, discharge summaries and progress notesAnswers all calls in a professional mannerGreets & shows respect to visitors and staff members
Qualifications
Minimum of a High School Diploma or GED requiredTwo (2) + years of healthcare/medical office experience strongly desiredHigh level of customer service experience requiredWorking knowledge of third party payers and insurance verification and authorization proceduresMedical terminology strongly preferredDemonstrates knowledge of governmental regulations regarding the confidentiality of patient informationStrong computer and internet skills requiredCall Center/phone experience helpful
