A EHE Community Health Worker contributes to AHF by utilizing a strong experience in care coordination with patient access and engagement. Within your duties of HIV testing, you have the opportunity to engage in outreach within the community and educate those at risk. You will support clients who are at risk of non-adherence. You will assist them in navigating the healthcare system and achieving positive healthcare outcomes. Utilizing your own community resource experience and a strong skill set in understanding and working through the healthcare system, you will work with new clients and at-risk members. Your compassion and ability to relate will comfort and trust the clients with whom you provide this care, engagement, and access to resources.

 

A EHE Community Health Worker enhances the reputation of AHF by Utilizing your experience in understanding the locality and the specific challenges facing this population to find creative solutions for re-engaging clients at risk of or have been lost to care will be pivotal in this role. The EHE CHW aims to keep clients engaged in care and understand their healthcare so healthy behaviors are achieved, resulting in a healthier population. With your creative skillset, you will work to achieve stronger engagement and health outcomes.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.

 

The EHE Community Health Worker will work as a part of a close-knit team to Have an understanding of the EHE Guidelines/Standards in Miami-Dade; you will collaborate with the Medical Care Management team, the Healthcare Center, Provider(s), and team and outside resources so each client receives access to available resources and the support required to be successful with their medical care.

 

Licenses and Certifications:

Bachelor’s Degree in Public Health specializing in Community Health, Social Services, Social Work, or a related field preferred.
Current HIV Testing Counselor Certification with a minimum of 6 months of work experience.
Bilingual (Spanish/English) or (Creole/English) are preferred.
Ability to work with minimal supervision.
Must be able to travel locally up to 90% of the time.
Sensitivity to the issues surrounding HIV and AIDS.
Must have reliable transportation and proof of insurance.
Must be able to drive a 26” Mobile Clinic
Must be able to work flexible hours, including evenings and weekends when needed.
Skill in communicating openly on sexual topics without prejudice or judgment.
