Overview

Continue CareRx Pharmacy provides a suite of services, including pharmacy, virtual nursing and pharmacist support, as well as coordination of health care resources such as home-based Primary Care, personal care services, home health and/or rehab. Whether patients are referred by a provider, a family or friend or after a discharge from a hospital or skilled nursing facility, we are with our patients every step of the way to ensure they receive the right care at the right time – when they need it most.

 

The Enrollment Coordinator works under the Continue CareRx, Director of Operations. Duties include assessing patient eligibility, patient education, and enrollment into the Continue CareRx Pharmacy. In-home visits conducted, as needed.

 

Qualified applicants must be:

Pharmacy Technician - Nationally Certified or Certified in the State 

LPN, LPT, LVN

Licensed Social Worker

 

Schedule: Monday - Friday 9am - 5:30pm ET

 

This is not a remote position based in the PharMerica Indianapolis, IN pharmacy. Applicants must live in or a commutable distance to Indianapolis.

Travel: 10%

Responsibilities Proactively contact eligible patients to introduce and explain the Continue CareRx Pharmacy Program, ensuring they understand its benefits and assisting with their enrollmentAddress patient inquiries, provide program-specific information, and ensure timely follow-up to ensure optimal patient satisfactionMaintain an accurate record of patient interactions and enrollment statuses within the system, ensuring all data is up-to-date and compliant with company standardsParticipate in monthly Branch Director meetings to provide insights, report on program performance, and offer feedback for improvementMonitor and track key program metrics (e.g., enrollment numbers, retention rates), identifying trends and ensuring program goals are being metReport on outcomes and suggest initiatives for enhancing program effectiveness, collaborating with leadership on strategic improvementsServe as the primary point of contact between branch operations, program leadership, and field clinicians, ensuring clear communication and alignment on program goals and patient careAssist with staff meetings, case conferences, and other relevant discussions where program input or patient care insights are neededProvide field clinicians with guidance and support, helping to troubleshoot challenges related to patient enrollment and engagementConduct in-home patient visits to facilitate the enrollment process where necessary, ensuring that patients who require additional support receive a personalized experienceCollaborate with field teams to coordinate visits, ensuring a smooth and efficient patient onboarding processDeliver outstanding customer service to both internal (branch leadership, clinicians, staff) and external (patients, families) customers in alignment with PharMerica’s valuesHandle any patient concerns or issues with professionalism and care, escalating to management when appropriatePerform other tasks as assigned by management, including special projects, as needed to support branch and program success Qualifications LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice  ORNationally certified Pharmacy technician in the state of practice ORLicensed Social worker with a degree from an accredited college with current unrestricted registration and license in the state of practiceValid driver’s license, acceptable driving record and proof of car insurance1+ years of medical related  experience preferredHome health care experience preferred but not necessaryAble to bend, stoop, push, pull, sit, and walk for periods of timeDetail oriented, goal oriented, team player, customer service, excellent oral communicationDemonstrates a strong commitment to providing exceptional customer service to both internal and external stakeholders, including patients and their families. Must be patient, empathetic, and capable of building positive relationships.Proven ability to work effectively with cross-functional teams, including leadership, clinicians, and administrative staff, to ensure high-quality patient care and program success.Ability to manage multiple priorities while maintaining accuracy and adhering to deadlines. Must be detail-oriented, ensuring all patient data is recorded accurately and promptly.Excellent verbal and written communication skills are essential, particularly in engaging with patients, explaining program details, and liaising with internal teams.Capable of identifying challenges and proactively offering solutions to improve program outcomes and patient satisfaction. Must be adaptable and able to navigate complex situations with professionalism.
