Responsibilities: 

Responsible for conducting pre-enrollment assessments for prospective enrollees utilizing clinical nursing expertise and critical thinking skills. Determines clinical eligibility and whether the prospect meets the criteria for services.  Utilizes a skillful approach for assessing, coordinating, and evaluating consumer needs and will need to develop a prospective plan of care.  Will be expected to complete pre-screening/assessment and complete appropriate paperwork.Assessments will be conducted in all service areas.Travel to all service areas.Will make plan to plan calls when indicated.Responsible for environmental assessments.Will complete semi-annual assessments.Participates in cross functional meetings. Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload to fulfill responsibilities. Utilizes a skillful approach for assessing, coordinating, and evaluating consumer needs and development of initial care plan. Responsible for clinical evaluation of all new participant referrals for appropriateness of therapy.  

 

Job Type: Full-Time

Salary: $90,000 to $109,000 per year

Qualifications

Qualifications: 

Current NY State Registered Nurse License 

Education:
