Summary:

The EP Device LPN promotes seamless care from referral to discharge within the Heart & Vascular Institute with a focus on Electrophysiology patients. The EP Device LPN works in direct collaboration with the cardiologists, cardiac surgeons, nurse practitioners, nurses and other team members; is responsible for the ongoing assessment of patients in the Electrophysiology clinic with and without devices. The Electrophysiology Device LPN participates in clinical decisions regarding the management of in-patients and outpatients; provides and promotes patient education; provides, promotes and encourages nursing staff education and involvement; and serves as a liaison between physicians, patients, families, nursing staff and referring facilities to promote quality and continuity of patient care.

Experience/Education Requirements:

Graduate of an approved LPN school of nursing.Extensive knowledge base in cardiac anatomy and physiology, cardiovascular physical assessments, arrhythmia and 12 lead assessment, emergency cardiac care, pathophysiology of coronary artery disease.Two years experience in coronary care unit preferred. Stress test familiarity a plus.


License or Certification Requirements:

Current IL LPN LicenseCurrent CPR certificationACLS certification within 6 months of hire into department preferred

 

Responsibilities:
Rounds on/reviews assigned patients regularly and evaluates patient progress with plan of care.  Communicates plan of care to patient and family and solicits concerns, questions, and issues for resolution.Records patient's health information and establishes/records care.Provides emotional support, counseling, clinical education and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision makingServes as a liaison between patient clinical staff, administration, physicians, managed care companies, and community/external resources to coordinate/maximize resources and identify/resolve barriers to the plan of care. Assists patients and families with resolving financial, psycho-social, functional, and administrative issues by advising of options and referring to appropriate resources.  Intervenes as appropriate to advocate for patient and family.Coordinate the complete delivery of care to cardiac device patients.  Perform EKGs; pacemaker and ICD checks in physician and nurse based device clinics.Assist in the coordination, scheduling, and performance of MRIs for patients with devices.Perform pacemaker and defibrillator checks with pacemaker programmers.  Document the device evaluations in the electronic medical record and pacemaker database.  Identify rhythm disturbances and/or device malfunction, as well as any other cardiac problem and report to the appropriate physician.  Performs device programming and troubleshooting with physician supervision.Provide remote pacemaker follow-up management and expertise with trans-telephonic monitoring.Assist in the development and implementation of per and post operative and discharge teaching for patients undergoing implementation of permanent pacemaker/ICD.Serve as contact person for patients/family members regarding device based questions.Provides device, arrhythmia and wound care teaching to patient/family members in the hospital before and after device implementation.Provides direct patient care by following practice policies and procedures to render appropriate clinical support to the electrophysiologist physicians in both the offices and the hospital environments.Responsible for accurate sample logging in EMR and medication room management.Ensures all clinical work queues including Rx authorization, and inbox messages are worked timely.Ensures clinic is in compliance wit DNV regulations.Follow up on lab results, prescription requests, referrals and patient inquiries in a timely and responsive manner with the appropriate health care team members and communicates with providers as necessary and required.Administers subcutaneous and intramuscular injections as requested by physician.Maintains patient privacy and confidentiality and protects operations in accordance with HIPAA laws.Maintains safe and clean working environment by complying with procedures, rules and regulations; calling for assistance from health care support personnel.
