ER and Critical Care Clinical Pharmacist

Description:Critical Care (CC) and Emergency Medicine (EM) staff pharmacists are essential members of the interdisciplinary healthcare team. They are responsible for optimizing pharmacotherapy regimens, ensuring safe patient care practices, and advocating for evidence-based medicine throughout the ED and CC units. MercyOne Des Moines is a 589-bed community teaching hospital, Level II Adult and Pediatric Trauma Center, Accredited Chest Pain Center, and Primary Stroke Center with a 16-bed Cardiac-Medical ICU, 16-bed Neuro-Trauma-Medical ICU, 16-bed Cardiac-Surgical ICU, 8-bed Flex ICU, 9-bed Progressive Care Unit and 56-bed adult/pediatric ED. The CC/EM pharmacist will rotate coverage between the various CC units and ED and between day and evening shifts as well as every fourth weekend.

Essential Key Job Responsibilities 

 Appropriately assesses patient data to identify medication-related problems through profile reviews, pharmacy consults and bedside interdisciplinary triage and rounding.     Participate in medical emergencies and other formal alerts such as trauma, stroke, sepsis and STEMI. Reviews and interprets physicians orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, and comorbidities. Checks for drug incompatibilities and allergies, and takes appropriate action when necessary. As necessary, consult with providers, nurses and other healthcare professionals involved in patient care regarding pharmacotherapy optimization with intent to achieve optimal patient outcomes across the continuum of care.   Provides accurate and timely responses to other healthcare professionals and patients regarding drug information questions and facility medication use policies to assure safe and appropriate use of medications. Serves as preceptor for pharmacy students/residents and assists with orientation, instruction and training of pharmacy students, interns, pharmacy technicians, residents, pharmacists and other health professionals. Assists nursing with medication administration (oral, injectable, continuous infusion titration) when necessary depending on the work assignments and in accordance with regulatory and hospital policies.  Documents patient care activities including therapy plan, monitoring and any follow-up in electronic medical record as per departmental guidelines.  Responsible for the completion of all assigned work during the shift, while maintaining the highest standards of quality and safety through the completion of quality control checks, identifying and reporting out medication errors and near misses, identifying process improvements and ensuring appropriate documentation.  Maintains current awareness and adheres to State and Federal laws, regulations, pharmacy policies, standard operating procedures, formulary and pertinent clinical practice guidelines while providing pharmaceutical care to patients.

Minimum Qualifications 

Required Education and Experience  

Pharmacy degree (BS or Pharm.D.) from an ACPE approved college of pharmacy 

Required Licensure and Certifications 

Licensed, or eligible for pharmacist licensure in Iowa; Board of Pharmacy Specialties (BPS) certification required within 2 years of hire; BLS, ACLS, and PALS required within 6 months of hire  

Required Minimum Knowledge, Skills, Abilities and Training 

Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years hospital pharmacy experience; completion of PGY-2 Critical Care or Emergency Medicine Pharmacy Residency or 3 years of experience in Critical Care or Emergency Medicine is preferred
