ESIM Sent - Community Services Coordinator

The Health Advocate works with team members in the development and operation of health and related activities that focus primarily on low-income and English-as-a-second language communities.  The Health Advocate is bilingual English/Spanish and demonstrates not only linguistic competence in both languages but also cultural competency and advocacy in his/her activities.  The Health Advocate takes the lead in community outreach, health education and advocacy.  The Health Advocate assumes increased responsibility and acts independently (within the team framework) in the development of program strategies goals, programs, and outcomes within the overall department plan.  Advocate engages community members in developing their leadership abilities.  He/she assists in the recruitment, training, and oversight of volunteers.

Required qualifications:

Bachelor's Degree Preferred fields: Health or Educational.  Degree from another country is valid or equivalent education/experience. Work experience related to program assessment, planning, development and evaluation. Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. Employment is contingent upon the completion of a language competency assessment that demonstrates competency in Spanish skills within 6 months 180 days of hire. 3 years' Experience in a healthcare setting or related position.

Preferred qualifications:

Coursework/Training: Formal or informal training in designated chronic diseases. Certification as a Promotora from a reputable organization (i.e. Latino Health Access) upon hire Experience in education or teaching. Experience in working with families and groups.
