Evidence-Based Care & Sepsis Coordinator (Horry/Georgetown Counties)

Position Summary: This position supports the integration of evidence-based practice (EBP) and research into clinical practice. Provide assistance and guidance in all activities that build a culture of inquiry and enhances the quality of care at Tidelands Health. Provides education related to Evidence Based Practice and research methodologies to facilitate collaborative and multidisciplinary research partnerships.

Develops, manages, and leads sepsis care coordination throughout the healthcare continuum by providing direction and oversight of sepsis care, throughout the enterprise. Leads the sepsis workgroup which reviews and evaluates current sepsis care, identifies care opportunities with current practice, develops a missed opportunity summary, implementation of new process solutions that improve sepsis care coordination and patient outcomes with providers, nurses and other hospital clinicians as needed. Serves as the clinical expert and resource for clinical staff, leads the sepsis workgroup, chairs the research and evidence-based practice council, and develops patient care protocols and co-leads the multidisciplinary team for Excellence in sepsis care in collaboration with the Medial Directors. 

 

The incumbent is expected to perform the following:

Facilitates the development and implementation of an evidence-based care strategic initiative across the organization in concert with the Director of Quality.

     - Demonstrates knowledge of federal regulations and regulatory knowledge for conducting research.

     - Serves as the principal champion and actively leads the Provation Order Set/Protocol Initiative, and is responsible for the development, refinement, and deployment of existing and new order set development processes to optimize efficiency and ensure safe high quality care delivery across the organization.

     - Builds collaborative teams, and mentors frontline clinical staff in the principals of the research process.

     - Participates in the implementation of system-wide education plan for evidence base practice.

     - Provides evidence-based practices and resources on the Quality and evidence-based care Intranet Bridge page.

Initiates, coordinates, and leads evidence-based practice initiatives in collaboration with Strategic Business Units, Clinical Directors, and ancillary support services.

     - Mentors frontline staff in the use of the approved Evidence Based Practice model and implementation of Evidence Based Practice

  -Disseminates Evidence Based Practice findings as appropriate.                                                                                                       

     - Participates in the implementation of system-wide education plan for Evidence Based Practice.

Facilitates use of current evidence to support clinical practices changes and improve quality outcomes as the co-chair of the Evidence-based Practice Nursing Shared Governance Committee

     - Searches and evaluates current literature to address existing practice gaps.

     - Facilitates frontline staff engagement in Evidence Based Practice through participation in nursing governance structure.

     - Supports frontline staff to make evidence-based decisions and or practice changes based on current literature.

Initiates, leads, and coordinates sepsis care across the healthcare system.

Leads the sepsis workgroup to review and evaluate current sepsis care.

Identifies opportunities with current practice, develops a missed opportunity summary report.

Implements new processes to improve sepsis care coordination and collaboration with providers, nurses and other hospital clinicians as necessary.

Serves as the clinical expert and resource for clinical staff on sepsis care.

QUALIFICATIONS

 

Education:  

- Bachelor’s degree in Nursing, Pharmacy, or related clinical specialty, required. 

- Master’s Degree in Healthcare Administration, Business, or clinical-related area, preferred.

 

Experience:

- 5 years’ acute care clinical experience, preferred

- Experience with developing clinical practice recommendations support by current literature, required.

- Experience leading quality initiatives, culture transformation, and performance improvement efforts, preferred.

- Experience in developing multidisciplinary collaborative partnerships, preferred.

- Research experience, preferred.

- Leadership experience, preferred.

 

License/Certification:

- Current & active license in good standing with the SC LLR, if applicable, required.

 

Skills:

Ability to communicate on a professional level and maintain good interpersonal relationships with physicians, nurses, other healthcare providers, patients and families is required.

Leadership skills to inspire and guide others toward goal achievement, being proactive and self-directed is required.

Strong data analysis skills are required.

The ability to prioritize, problem solve, build a collaborative climate for teamwork and building consensus are required.

Proficiency with Microsoft Office software programs such as Outlook, Word, PowerPoint, and Excel are required. Power BI experience is preferred.

Excellent interpersonal, communication, presentation, conflict resolution and team facilitation skills.

Ability to lead and coach decision makers regarding best practice and evidence-based practice approaches

Prior Meditech and Provation software experience, preferred.
