Position Summary

That’s why we are creating connected, community-based health care experiences focused on prevention and better management of chronic disease.  Through our Health Care Delivery channels which include primary care, retail health clinics, home health services and provider enablement capabilities, we are driving improved outcomes, enhanced patient experiences, better access to quality care, and reductions in care costs.

Our Healthcare Delivery ACO is recruiting for an Executive Director, Clinical Health Services who will provide strategy, executive leadership and oversight of all Care Management operations including clinical program delivery and call center operations. This leader will collaborate across multiple departments and with external partner organizations to coordinate and facilitate comprehensive care for individuals by assessing their needs, develop personalized care plans, and coordinate services across health care providers. This leader will inspire the team to serve as advocates for patients, ensure effective communication, appropriate resource utilization, and continuous monitoring of their progress to promote positive outcomes, enhance overall well-being, and reduce total cost of care.

You’ll make an impact by:

Maintaining oversight of all inbound and outbound outreach, and all clinical products implementation and services delivery.

Executing strategic and operational plans in support of business segment services objectives and initiatives.

Setting business area priorities; allocating resources and developing plans for multiple related teams.

Ensuring all critical service metrics and operational results are achieved. Leading multiple leaders of a highly customized operations.

Potential to oversee multiple service sites of delivery.

Directing implementation of service standards for each location to ensure delivery of evidence-based, clinically accurate, quality-focused, consistent, cost-effective services and administration.

Analyzing operational practices for effectiveness and practicality, while creating a culture which is innovative in its approach to solutions.

Responsibility for all Care Management operational and effectiveness metrics.

Establishing a clear vision aligned with company values, including health equity; setting specific challenging and achievable objectives and action plans; motivating others to balance customer needs, budgets, and business success.

Developing an organization that attracts, selects, and retains high caliber, diverse talent able to successfully achieve or exceed business goals; building a cohesive team that works well together and across other business segment functions.

Coordinating major plan modifications necessitated by unanticipated business or technology developments.

Overseeing development of a network with community health, ambulatory and population health partners in efforts to support patients across the care continuum.

Directing activities to implement approved work methods, patient care models and procedures that reflect elements essential to rendering high quality care.

Establishing infrastructure to support clinical record reviews, ensuring all documentation and workflows are completed within established timeframes and demonstrating appropriate clinical quality.

Approving appropriate suggestions from clinical managers and other individuals on matters pertaining to Care Management operations and external relationships.

Developing and maintaining executive level relationships with ACO partner organizations and coordinating with their services through contract and workflow agreements.

Representing the work of the ACO clinical teams at internal and external executive and c-suite level meetings. Managing relationships with external agencies, including governmental and third-party payers or qualified representatives.

Required Qualifications

The candidate will have a strong work ethic, be a self-starter, and be able to be highly productive in a dynamic, collaborative environment. This position offers broad exposure to all aspects of the company’s business, as well as significant interaction with all the business leaders. The candidate will be expected to have the following key attributes:

10+ years of work experience with progressive levels of responsibility and senior management experience in care management and/or clinical operations including but not limited to call center management.

5+ years of clinical experience delivering hands-on care.

Highly developed and demonstrated use of healthcare data and analytics.

Ability to work successfully in a fast-paced, matrixed environment while building high-performing teams.

Adept at problem solving and excellent decision-making skills.

Executive presence and presentation skills.

Mastery serial tasking abilities with the ability to handle competing deadlines; flexible and adaptable.

Ability to deliver professional level customer service.

Mastery execution and delivery (planning, delivering, and supporting) skills.

Demonstrate a commitment to diversity, equity, and inclusion through continuous development, modeling inclusive behaviors, and proactively managing bias.


Preferred Qualifications

Advanced clinical degree – APRN and/or master’s degree in related field.

Proficiency with EHR (Epic).


Education

RN/BSN or higher nursing degree required. Master's Degree in related field or equivalent work experience.

Pay Range

The typical pay range for this role is:

$184,112.50 - $396,600.00


This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.  This position also includes an award target in the company’s equity award program.
