Provide inpatient and outpatient Palliative Medicine consultation services. Consult with the attending physician and the Interdisciplinary Team to contribute to the patients plan of care. Facilitate clarification of patient and family goals of care. Provide expert physical and non-physical symptom assessment and management. Facilitate access to appropriate supportive care services. Participate weekly in interdisciplinary team meetings to review active cases. Appropriately document patient care services to support professional billing for these services. Serve as collaborating physician for Palliative and Supportive Care Service nurse practitioners. Collaborate with Hospice Medical Director to help provide mutual medical coverage in case of eithers absence. Participate in Quality Improvement Initiatives through the Center of Palliative Medicine. Assist in the development of standards of care through the Center of Palliative Medicine. Provide Palliative Care education for staff, colleagues, students, and community. Perform other duties as requested. Adhere to the practice of confidentiality regarding patients, families, staff, and the organization. At this time, no after-hours Palliative Medicine on-call coverage is expected, though with service growth this could be a development in the future.
