POSITION SUMMARY:
The person in this position will be responsible for general client supervision, milieu management, client transportation, meal preparation and general program operation. The Facility Monitor/Peer Recovery Coach can facilitate house meetings but will not provide curriculum-based group unless trained to do so. In addition, this position will provide strength-based support for persons in or seeking recovery from behavioral health-related issues. The service offers a type of partnership where the person in or seeking recovery self directs their care while the Peer Recovery Coach provides expertise in supporting successful change. The Peer Recovery Coach focuses on achieving any goals important to the individual and asks questions and offers suggestions to help the person begin to take the lead in addressing their own needs. The practice focuses on honoring values and making principle-based decisions, creating a clear plan of action, and using current strengths to reach future goals. The Peer Recovery Coach serves as an accountability partner to help the person sustain their recovery and helps to access the systems needed to support the individual, such as disability benefits, health care, housing, etc. Essential Job Functions include those listed below.
ESSENTIAL JOB FUNCTIONS:
Overall supervision of clients in the program including ensuring safety, well-being, and compliance with program rules and guidelines. Create a positive and supportive living environment within the program including ensuring a structured atmosphere, facility cleanliness, promote healthy interactions among clients, and address any conflicts or issues that may arise. Provide transportation for clients to appointments, job interviews, or other necessary outings and ensure the safe transportation of clients and follow established protocols and guidelines.
Assist with meal preparation and ensure that nutritious meals are provided that accommodate dietary restrictions or special meal plans as directed by program guidelines or staff. Support the overall functioning of the program including administrative tasks, maintaining documentation, coordinating with other staff members, and participating in team meetings. Facilitate house meetings, coordinates and facilitates psychoeducational peer groups related to removing environmental obstacles and improving healthy life skills and social connectedness including teaching relaxation and mindfulness strategies. If trained in curriculum-based group facilitation, lead or assist in conducting group sessions focused on specific therapeutic topics or interventions. This responsibility would require familiarity with the curriculum and the ability to effectively guide clients through the material. Encourage, inspire and empower residents to reach treatment goals Works as part of a collaborative health team and participate in care team meetings Assists individuals in developing strategies to communicate with and advocate for themselves to adopt a proactive role in their own behavioral and physical health Instructs about services provided to help them (e.g., availability of peer services, benefits, advanced directives, living will opportunities, clients rights process) Mentors in the recovery process (e.g., reassures to the reality of recovery, assists with increasing personal empowerment, helps set & achieve goals) Provides information and assists with applying for supplemental programs such as housing Assists persons with completing forms; develops services and supports to identify service and support gaps for individuals with mental health or co-occurring mental health and substance use disorders; participates on treatment teams with person’s consent; Collaborates to assist with intake/discharge planning (e.g., conducts intake and discharge surveys, facilitates exit interviews) Monitors satisfaction Completes and submits documentation as determined by the Program Director Provides support and encouragement to family members Meet individuals where they are in the community and transport individuals to appointments as needed Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and Perform all other duties as assigned.
