Job Summary: The Family Case Planner will work in a child welfare prevention program that utilizes the Solution Based Casework model to stabilize families who have children under the age of 18 that are at risk for placement outside their home.
Direct Services are provided to clients via casework, individual, family, and group counseling, as indicated by individual case assessments. Assist in Intake and Crisis Intervention. Conduct home visits and attend client-related meetings as needed. Develop client assessments and service plans following the model outlined by Improved Outcomes for Children, Family Team Conference, and SBC. Monitor and assist in service provision to all program clients as required by program's mandates. Maintain current case records and service plans (FASP) for families served utilizing Connections and PROMIS systems. Participate in weekly supervision, case conferences and Family Team Conferences as directed by Supervisor and Program Director. Provide parent education and other support services utilizing group format as determined by client need. Participate in training appropriate to the development of enhanced Social Work skills as required by Children’s Services and HeartShare St Vincent’s Services. Assist in day-to-day operations as required by the Program Director.
