Our mission is to create a safe and nurturing environment where children and youth can thrive and families can grow stronger together with the help of their community. Your expertise matters, and together, we can create positive change." - Susan Bellas, Hiring Manager  Purpose: As our Family Health and Wellness Coach you will work with families in their homes and you will teach, coach and support children, teens and parents with significant mental health conditions in learning new skills and strategies practicing skills in their homes and sustaining these skills: parenting skills, coping skills, emotional regulation skills and more.  Job Summary: In this position you will guide clients with significant mental health and co-occurring substance use in behavioral strategies to improve overall health. This position will use psycho-education and psycho-social rehabilitation interventions to keep clients as healthy as possible. You will work with children, teens and parents with high needs to improve their skills in emotional regulation, crisis management, parenting strategies, relationship skills and others. You will also contribute to creating goals and objectives on care plans or develop care plans in full when Health and Wellness Coaching is the only service engaged. Job Profile: Do you love working with teens and children? Are you able to provide services in a coaching approach that is warm, nonthreatening, non judgmental? Do you have experience from your past to draw on regarding, parenting, substance use, mental health, or public health you can uses to help influence clients to connect and engage? Have you taught parenting skills with a challenging population? Are you looking for a role where you are out in the community, connecting with clients in their homes? You will report to the Program Manager Roughly 70% of time is spent on Health and Wellness Coach duties, 20% on Care Coordination duties, and10% on Client Service Advising duties.
