Full-Time Family Health Worker - Internal Medicine - Queens Reports To: Head Nurse 131-72 40th Road Flushing, New York, 11354 United States Responsibilities : + Coordinate quality assurance activities related to unit’s services and projects + Responsible for tracking, coordination, communication, and documentation of patient referrals to ensure referrals are addressed in a timely manner + Provide information and instructions to patients pertaining to their referrals and specialist appointments + Work closely with directors, unit managers and staff on clinical matters, including clinical staff training + Work closely with Health Center departments and outside agencies on matters pertaining to clinical systems, such as EMR, Quest, Bellevue, NYP-LMH, etc. + Serve as liaison between patients and providers to address issues/concerns pertaining to referrals and prior authorizations + Serve as point of contact for patients and caretakers so they can have direct access to support for their referral questions/concerns + Serve as referral resource center for staff + Liaison with community agencies related to unit’s projects + Assist in completing administrative reports, compiling statistics on program activities, and keeping track of all completed activates as part of programs review + Supervise Family Health Worker activities + Perform other duties assigned Minimum Qualifications : + Must be bilingual in English/Chinese + Two years college or equivalent experience + One to two years community experience + Knowledge or experience in healthcare preferred + Strong customer service focus + Strong computer skills Compensation and Benefits : + Salary: $40,000.00 - 46,000.00 per year + Medical insurance + Employer-paid dental, vision, disability and life insurance + 403(b) retirement savings plan + Flexible spending accounts for transit, parking, health and dependent care + Paid time off (28 days for the first year of employment) + Employee Assistance Program + Tuition reimbursement for eligible employees, and + The Health Center is qualified for Public Service Loan Forgiveness (PSLF) program
