This role involves assessing client needs, developing individualized service plans, and connecting clients to housing resources and social services to enhance their quality of life. Duties and Responsibilities + Conduct intake interviews and assessments to identify clients’ needs, strengths, and barriers related to housing and support services. + Develop and implement individualized service plans, outlining goals and objectives tailored to each client's circumstances. + Connect clients with housing resources, healthcare services, mental health support, and other community resources. + Advocate for clients in accessing services and navigating systems, ensuring they receive the support necessary for stability and well-being. + Regularly monitor client progress towards goals, making adjustments to service plans as needed. + Maintain accurate and up-to-date client records, including case notes, service plans, and progress reports, in compliance with organizational policies and local, state, and federal regulations. + Work collaboratively with other service providers, community organizations, and government agencies to enhance service delivery and ensure comprehensive support. + Provide education to clients and the community about HOPWA services and resources available for individuals living with HIV/AIDS. + Performs related work as assigned. Education, Experience, Licensing Minimum Requirements: + Requires any combination of education and experience equivalent to an Associate's Degree or specialized courses/training equivalent to satisfactory completion of two years of college in human services or a closely related field and two years of psychology, social work, case management, human services or closely related experience. + Possession of a valid driver’s license with an acceptable driving record required. Preferred Requirements: + A bachelor’s degree in psychology, social work, or human services + A bachelor’s degree in any field of study with at least six months of case management experience + Applicable experience, which may be substituted on a year-for-year basis for the required education + A registered nursing license in Florida. Supplemental Information The work schedule is Monday to Friday 8:00 a.m. to 5:00 p.m. May require occasional evenings or weekends.
