The Family Liaison/Tech Navigator is tasked with actively engaging with parents and caregivers, offering comprehensive support both at home and within community environments.

 

This role involves working with a multidisciplinary team to provide personalized family assistance, access to resources and referrals, and delivering coaching, training, and educational services to families and participants. They will empower families and individuals by facilitating access to and proficient use of modern technology, including smartphones, tablets, and various applications to increase personal independence, oversee health management, and accomplish ISP objectives.

Responsibilities Responsibilities:

Facilitate Connections: Link families and participants to community-based services and natural supports.

Advocate: Represent the family's and participants' needs and desires within the multidisciplinary team.

Support Caregivers: Help parents and caregivers gain tools and strategies to manage their child’s behavioral and health needs, aiming to reduce caregiver fatigue and enable participants to live at home.

Cultural Sensitivity: Demonstrate cultural awareness and possess strong linguistic skills.

Coaching and Mediation: Provide informal coaching and assist in cultural mediation.

Care Planning: Collaborate with the team to develop and monitor interdisciplinary, person-centered, holistic care plans.

Education and Resources: Offer ongoing education and resources to participants and caregivers regarding health and safety.

24/7 Consultation: Participate in a rotating schedule to provide 24-hour consultation calls with caregivers and participants.

Assessments and Plans: Complete assessments and develop health care plans.

Confidentiality: Maintain confidential health records.

Documentation: Ensure timely and accurate completion of all documentation and reports.

Liaison: Coordinate with families, caregivers, and other providers to meet ongoing health care needs.

Assistive Technology Training: Provide training on assistive technology for families and participants.

Training and Certification: Complete and maintain all required training, certifications, and licensures.

Privacy and Security: Protect the privacy and security of individual, family, and organizational information, ensuring compliance with HIPAA and other regulatory standards.

Promote Values: Actively promote and role model the organization’s philosophy, mission, and core values, treating all stakeholders with respect and dignity.

Safety and Maintenance: Respond effectively to safety hazards and emergencies, maintain a clean and safe environment, and adhere to maintenance and safety protocols.

Flexibility: Work flexible hours as required.

  Qualifications

High school diploma or GED required.

 

Requires moderate computer skills, including experience working within electronic health records (EHRs).

 

Strong written and interpersonal communication skills, and cultural competence.

 

Strong understanding of human rights, self-determination and individualized treatment planning.

 

Ability to work as part of a multidisciplinary team.
