Job Title:Nurse Practitioner / Physician Assistant – Primary Care Location:130 N Sherman Street Leslie, MI 49251 Department:Primary Care Inpatient/Outpatient:100% outpatient(applicant must be able to see patients of all ages) Position Overview: Henry Ford Health Jackson is seeking a dedicated and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our dynamic Family Medicine team inLeslie, MI. This full-time position requires at least two years of experience as an Advanced Practice Provider (APP) –no newgraduates will be considered. The role involves providing comprehensive care to patients in a family medicine setting, focusing on preventive care, chronic disease management, and acute care. The ideal candidate will have a strong clinical background in family medicine, excellent communication skills, and a commitment to delivering high-quality, patient-centered care. Schedule– Flexible. Call requirement– 1 week per calendar year. GENERAL SUMMARY: Provides inpatient and/or outpatient medical care. Takes patient history, performs physical examination, orders, and interprets tests, makes diagnoses, and institutes appropriate treatment programs. Works in collaboration with staff physicians, house officers, nurses, and other medical care team members to ensure that patient outcomes are achieved safely and efficiently. Specific procedures are individually credentialed as indicated by training and experience. EDUCATION/EXPERIENCE REQUIRED: Master's degree in nursing. CERTIFICATIONS/LICENSURES REQUIRED: Certified Nurse Practitioner. Licensed as a Registered Nurse. Current BLS or ACLS as required. Current ATLS/PALS provider status as required by department. Qualifications: · NP – Completion of a Nurse Practitioner training program accredited by the Commission on the Collegiate of Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC) · PA – Completion of a Physician Assistant training program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) · NP – Certification by a certification body recognized by the State of Michigan · PA – Certification by National Commission on Certification of Physician Assistants (NCCPA) Licensure and Certification · Current licensure as a Nurse Practitioner or Physician Assistant in the state of Michigan · National certification as an NP or PA (e.g., ANCC, AANP, NCCPA) · Advanced Cardiovascular Life Support (ACLS) certification required. · Basic Life Support (BLS) certification required. · DEA/CSL, required through credentialing. · DEA - NPs do not have controlled substance licenses, only PAs. When NPs apply, they will enter MI license number instead of controlled substance license since they won't have a CSL. Experience · Minimum of 2 years of experience as an APP. · EPIC Experience, preferred. Skills · Strong clinical assessment and diagnostic skills · Excellent communication and interpersonal skills · Ability to work effectively in a fast-paced, multidisciplinary environment. · Commitment to patient-centered are and continuous professional development.
