General Functions:  

The Family Navigator-CSNF will work to reduce unnecessary placement and expedite safe placement permanence of Alaska Native and American Indian children and youth families through coordination of support and resource linkages for parents, children and youth through the OCS Team Decision Making meeting process.  The Family Navigator-CSNF serves as the initial point of contact for new referrals to the program, coordinates communication with OCS regarding new referrals, guides participants through the enrollment process, provides pre and post-meeting assistance to family members, and coordinates expedited access to essential resources, particularly an array of recovery services, within CITC’s service areas and within the Alaska Native system of care.

Duties and Responsibilities, included but not limited to: 

Provide orientation and navigation assistance to parents, youth and children prior to an OCS Team Decision Making meeting to reduce stress and to improve meaningful participation.Provide emotional support and explain, reiterate, or manage the pace in OCS Team Decision Making meetings for family members.Conduct intake, enrollment, and initial screening and assessment surveys with new participants.Assist parents to develop an informal support network of family members and other supportive people to function in various capacities including safety plan participants and licenses or un-licensed placements to manage the safety of children.Coordinate access to services to promote change that supports safety and well-being of children.Document activities for purposes of service provision and evaluation activities.Provide planning and facilitation support for parent-child dyad connection activities for parents and children in the program.Coordinate and provide transportation for participants and their children to parent-child dyad connection activities as needed.Maintain operations by following policies and procedures.Maintain professional and technical knowledge by attending workshops; reviewing professional publications; establishing personal networks.Comply with Federal, State and local requirements by studying existing and new legislation; enforcing adherence to requirements; advising management on a needed basis.Maintain client confidence and protect operations by keeping information confidential.Contribute to team effort by accomplishing related results as needed.

Responsibilities Related to Patient Privacy

The incumbent is expected to protect the privacy of all patient information in accordance with CITC’s privacy policies, procedures and practices, as required by federal and state law, in accordance with general principles of professionalism as a health care provider.   Failure to comply with CITC’s policies and procedures on patient privacy may result in disciplinary action up to and including termination of employment or association with CITC.The incumbent may access protected health information and other patient information only to the extent that it is necessary to complete your job duties.  The incumbent may only share such information with those who have a need-to-know specific patient information you have in your possession to complete their job responsibilities related to treatment, payment or other CITC operations.The incumbent is encouraged and expected to report, without the threat of retaliation, any concerns regarding CITC’s policies and procedures on patient privacy and any observed practices in violation of the policy to the designated Privacy Officer.The incumbent is expected to actively participate in CITC privacy training and is required to communicate privacy policy information to coworkers, students, patients and others in accordance with CITC policy.

Minimum Qualifications:

Bachelor’s degree in human services, social work, or related field.  Relevant experience may be substituted for the education requirement on a year-for-year basis.Minimum of two full years of work experience and proficiency with knowledge of the OCS system and other services systems accessed by participants.Minimum of one year experience working with Alaska Native/American Indian families and knowledge of their values and belief systems.Valid Alaska driver’s license and insurable under CITC’s automotive insurance, which requires a driver to be at least 21 years of age and have had a driver’s license for at least three yearsContinued employment is contingent upon completion of a satisfactory state and federal fingerprint criminal background check.

Physical Requirements:

Primarily works in an office setting, with extended periods of time at a desk and on a computer.Must be able to lift up to 50 pounds.
