Qualifications

Education: 

Graduate of an accredited school of medicine requiredCompletion of an accredited residency program requiredFor specialty physicians - completion of an accredited fellowship program specific to medical specialty

Licensure/Certification/Registration:

Current MD or DO license in appropriate stateBoard certification or eligibilityCurrent DEA license required Responsibilities
