Position Overview The Family Support Specialist delivers assistance and supportive services to Veteran participants with children who are enrolled in the SSVF program. They provide care coordination ranging from start-up activities in the SSVF program for all household members to referrals for community-based specialized services, as well as ongoing family-oriented support to participant families and staff assigned to their cases. This staff member provides comprehensive benefits/income and service linkages and reviews for households in the SSVF program, enhancing the program’s service delivery for families, expanding the coordination and use of eligible child-care services, and assists team members in the tracking and delivery of daily services, including providing additional engagement capacity to program and various administrative supports.

 

ESSENTIAL DUTIES & RESPONSIBILITIES:

Empower program participants to achieve their families’ housing stability goals in conjunction with SSVF social service team and participant families. Assist Veteran households with accessing needed family-focused community services, including but not limited to public benefits such as SNAP, WIC and TANF, clothing, legal & child care services, educational and training resources, transportation assistance, and other supports through the development of a referral network and close follow-up.Assist Veteran households with accessing needed McKinney-Vento services for all at-risk/homeless children in the household who may be entitled to transportation and other accommodations related to their housing crisis. Complete the SOAR program training and certification, and use that system to process households’ SSA applications.Enhance the program’s outreach ad engagement to veteran families with children and expand  key linkages with community stakeholders who serve veteran families. Seek out, foster and interface with representatives from referring agencies in order to facilitate the delivery of program services, tracking referrals as needed.Develop, assess and monitor individualized housing stability plans in conjunction with SSVF staff.Track program milestones, including HSP completion dates, re-certifications and retention data as directed by program management. Maintain detailed benefits and referral outcome records.Work with the case management, employment and outreach teams to navigate participants to community resources. Assist with administrative functions of the housing search and placement process (i.e. data entry, filing leases, W9 forms, organizing charts, delivering checks, setting up utilities, ordering household items, etc.).  Coordinate access to the programs’ Health Navigators and community-based supports for children and families. Follow-up with households to assess their ongoing housing stability needs. Assist program management in regular participant record reviews. Document all services in compliance with agency standards.Participate in ongoing individual supervision, team meetings, case conferences, division meetings and trainings.Respond to information requests from internal S:US staff.Complete other duties as assigned by supervisor. Qualifications

REQUIRED EDUCATION AND EXPERIENCE

Bachelors’ degree preferred. Previous experience supporting the needs of families with children in a human services or related field strongly preferred. Experience with electronic database systems (such as HMIS) is a plus.Ability to work with families across Nassau and Suffolk counties of Long Island, with occasional work in the five boroughs of NYC. This role is based out of the Amityville SSVF office.  

PREFERRED QUALIFICATIONS & SKILLS

Demonstrated proficiency with Microsoft Office Suite required.Solid organizational skills and detail oriented.
