Responsibilities

MJHS Home Care provides advanced clinical services and emotional support to patients recovering from illness, injury, or surgery. With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home.

Someone with your depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient’s recovery from illness, injury or surgery at home.

You will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family educationYou will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of your patientsYou will review patient insurance information and coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers Qualifications Current NYS RN license and registration requiredBachelor’s degree in nursing preferred; Associate's degree requiredMinimum of one year acute medical-surgical nursing experience requiredCertified Home Health Agency (CHHA) experience preferredValid Driver License and CarBilingual English and Spanish or Mandarin or Cantonese preferred

Fee for Service Visit Rates:

 

Start of Care Weekend/Holiday Visit Rate: $120 (Maintain the premium for 3+)

Start of Care Weekday Visit Rate: $100 (Maintain the premium for 3+)

Revisit Rate: $85

Orientation Daily Rate: $250

In-Service Hourly Rate: $55
