Duties and Responsibilities + The Field RN (Registered Nurse) performs home visits, assessments and follow up visits to patients. + The RN advocates for quality patient care, collaborates with Physicians and other providers to maintain a safe and effective home environment for patients. + The Registered Nurse works closely with patients, families and interdisciplinary team to ensure quality care is provided at home. Job Qualifications Required + Must be a NYS Registered Nurse + Experience working for a CHHA or Prior Hospital Med/Surg experience + Ability to visit patients in a home environment + Willingness to collaborate with interdisciplinary team as needed + Excellent organizational skills required Education, Knowledge, Skills and Abilities Preferred + Knowledge of ICD 10, Medicare/Medicaid and OASIS guidelines a plus + Ability to drive and have access to reliable automobile preferred + Bilingual Chinese, (Cantonese/Mandarin) Russian, or Spanish a plus Job Type: Fee for Service - Shifts based on your availability and location preference!
