Job Summary: An organized, directed, postgraduate 12-month training program that centers on the development of the knowledge, attitudes, and skills needed to pursue rational drug therapy. It is essential for pharmacy graduates to obtain the level of knowledge and skills needed to become competent in pharmacy practice. Pharmacy residencies serve as one of the best means by which to achieve that objective. This residency will provide the resident with opportunities to conceptualize, integrate and transform accumulated experiences and knowledge into improved drug therapy. This residency will instill a philosophy of practice that embraces the concept that pharmacists must be responsible and accountable for optimal drug therapy outcomes. Essential Responsibilities: + Training and education in the provision of pharmaceutical care. + Directly participate in the development, implementation, provision, evaluation and communication of pharmaceutical care and pharmacy clinical services. + Participate and Coordinate health system-wide and Departmental programs and projects under the direction of the residency preceptor. + Experience will include Ambulatory care, drug information and drug-use policy development, management and acute care across and integrated health care system. + Other duties as requested. Basic Qualifications: Experience + N/A Education + Graduate or candidate for graduation from an Accredited Council for Pharmacy Education (ACPE) accredited degree program (or one in the process of pursuing accreditation) or Degree in pharmacy meeting minimum educational requirements for pharmacist licensure in Oregon or Foreign Pharmacy Graduate Examination Committee (FPGEC) certificate from the National Associations of Board of Pharmacy (NABP) upon hire/transfer License, Certification, Registration + Pharmacy Intern License (Oregon) required at hire OR Pharmacist License (Oregon) required at hire + Pharmacist License (Washington) within 6 months of hire + Pharmacist License (Oregon) within 4 months of hire Additional Requirements: + Must be a licensed Oregon pharmacist within 120 days from the program start date. Residents who are not licensed as a pharmacist within 120 days from the program start date will have their term of appointment extended by the number of days the resident is without licensure past the 120-day deadline up to a maximum of 45 days. Failure to obtain a Oregon state pharmacist license within 165 days from the program start date will result in the resident being dismissed from the program and employment will be terminated. + Must receive a Washington Pharmacist license within 180 days of start of program. + Completion of an Accredited PGY1 Pharmacy residency program (ASHP) prior to start date. + Must participate in and adhere to the rules of the Resident Matching Program (RMP) or the Early Commitment process. Preferred Qualifications: + Doctor of Pharmacy degree or clinical Masters Degree. + Knowledge of integrated health care systems. + Knowledge of pharmacotherapeutics and application to program development.
