Field Based Patient Care Coordinator - Multiple Locations

Growing together.** As a **Patient Care Coordinator,** you operate as an Optum ambassador. Your primary responsibilities include data entry and uploading documents into our EMR and SNF EMR, obtaining info from area hospital EMR. You will also provide operational support, including backfill and care coordination activities. You will put emphasis on how to manage shifting priorities and attention to detail. There will be collaboration and communication across the "Team Model“ with the SCC member in the center. Key systems that you will use on a regular basis are Outlook, Salesforce/Pathway EMR. Valuable skillsets to have in order to be successful in the role is include strong organization skills, multi tasking, ability to sort excel You will set up a home office for project work and will also be working with their team in the SNF, which will involve a lot of customer facing work. You will be answering inbound calls and making outbound calls. And you will work both independently and collaboratively – working with the RN’s and Nurse Practitioners.. And can always call upon the assistance of your Department Supervisors and Managers, when needed. You will be part of the primary care team collaborating with all interdisciplinary team members and will support multiple providers. The Patient Care Coordinator is a flexible team member who works to support nurse practitioners (NPs) and physician assistants (PAs) as well as our patients. This role assists in care coordination activities, administrative tasks and supports our patients and providers. This position is full-time. Employees are required to work our normal business hours of 8:00am – 5:00pm, Monday – Friday, no nights, weekends or holidays. It may be necessary, given the business need, to work occasional overtime. We offer 2 months of paid training. Knowledge of medical terminology and EHR platforms + Access to reliable transportation that will enable you to travel to patient locations + Must reside in Milwaukee, Racine, Kensoha, or Waukesha, WI + Ability to work our normal business hours of 8:00am – 5:00pm, Monday – Friday, no nights, weekends or holidays. It may be necessary, given the business need, to work occasional overtime **Preferred Qualifications:** + Certified nursing assistant or medical assistant certifications preferred **Telecommuting Requirements:** + Ability to keep all company sensitive documents secure (if applicable) + Required to have a dedicated work area established that is separated from other living areas and provides information privacy + Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service The hourly range for this role is $19.86 to $38.85 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
