Job Description:

The Clinical Pharmacist demonstrates consistent application of knowledge and skills to deliver appropriate, safe, efficient, effective, and cost effective pharmaceutical care in the scope of their particular assignment. This position is responsible for the care of patients as well as the development of effective, professional relationships within the multidisciplinary team.

Schedule

This is a benefited 0.5 FTE position. The 0.5, or 40-hours per pay period, are guaranteed. However, if there are more shifts available, more can/will be assigned. The shifts can be between 7:30am-9:30pm M-F, 8am-6pm weekends. This is a shift in the Talent Share Float Pool, so the shifts can be at any Front Range Outpatient Pharmacy (SJH, GS, PLH).

Scope

1.Accurately dispenses prescription medications. Reviews patients medication order forallergy interactions, duplication of therapy, appropriateness of drug therapy, and drug- drug interaction. Makes appropriate evidence-based interventions to optimize medication use. Ensures safe, appropriate, cost effective medication therapies for SCL Health patients.
2. Provides counseling to patients and promotes quality medication education and adherence. Ensures patient is well informed, treated with respect, and receives the highest level of professional level service. Ensures prescriptions are accurately billed and dispensed to the patient.
3. Supervises and directs activities of interns, technicians, and support personnel as required. Provides medication related clinical consultation and clarification to other health professionals. Participates in the evaluation and selection of formulary medications.
4. Performs in an efficient, professional manner to meet pharmacy profitability and productivity targets. Ensures compliance of the pharmacy with state board regulations.
5. Takes the initiative to solve problems including, but not limited to, equipment, maintenance, drug availability, staffing issues, and extraordinary patient care concerns. Participates in quality and patient safety initiatives. Identifies potential areas for improvement including processes that could be streamlined or revised to improve patient satisfaction. Identifies potential safety hazards that may create problems for patients and/or staff, and takes appropriate actions. Participates in departmental meetings, contributing ideas or feedback.
6. Supports inventory and stewardship initiatives.

8. (All SCL jobs) Performs other duties as assigned.
9. (If applicable) May be required to float to other departments (within scope of competency and qualifications) based on business need.
10. (If applicable) May be required to be placed on-call during a regularly scheduled shift.

Minimum Qualifications

Required
Graduate of an accredited College of Pharmacy with a five-year Bachelor of Science Degree or Pharm D required
Current licensure by the state of practice in Pharmacy is required BCLS certification is required Immunization certification required


Preferred

Board certification in pharmacy practice or other recognized BPS specialty is preferred MTM (Medication Therapy Management) cert is preferred
One (1) year of experience in a outpatient or retail Pharmacy is preferred

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$55.00 - $91.64
