Job Summary:
The role of the Foster Care Specialist is to be a vital member of the Youth treatment team by supporting youth and foster parents to maintain stability, well-being, safety in the home while also supporting efforts towards the youth’s goal of permanency. The Foster Care Specialist serves as a liaison between treatment team members and lead the treatment planning process.
Essential Job Functions:
Treatment Planning and Service Delivery:
By actively engaging all treatment team members including, but not limited to family of origin, foster parents, DCS Family Case Manager, previous/current service providers, teachers, DCS Ed. Liaison, Mentors, Guardian Ad Litem, CASA, and other identified supportive adults or child representatives. The TCS will lead the treatment planning process upon youths’ admission to a System foster home. The TCS will develop the Treatment Plan with input from the treatment team and staff with HSPP.
Provides support and consultation to foster parents, families of youth in care, and to other treatment team members related to their role as described in the treatment plan.
Coordinates and supports the integrated care of the youth
Provide crisis intervention/emergency services:
By assessing risk, development of safety plans, reporting of any abuse or neglect, or any other emergency situations.
Communication and collaboration with collateral contacts, stakeholders, and referral sources; Develop and maintain effective relationships:
By consulting and/or meeting with providers and stakeholders regarding client care, integrated health, current function, medication related- issues, education support, and any other care related to client/families. Maintaining open and professional communication with Team Leaders, clients/families, and staff/team members.
Accountability and Documentation:
By completing documentation timely and accurately, maintaining ethical standards of the System, maintaining proficiency in the EHR, meet contractual and reporting obligations for referral sources and governing bodies by required timelines
Participate in training and development:
Complete all required trainings, attend clinical staffing, active participation in supervision and other recommended trainings; to provide professional development
Creating a professional and ethical environment:
By promoting open communication, ethical practice and building upon the strengths of the team
Meet the metrics assigned to this position:
Meet service delivery: By meeting the expected service delivery of 60%; Compliance with required amount of visits with youth and foster parents per week.
Note Timeliness: By submitting notes within policy and procedure guidelines 95% of the time, as well as having staffing records, CANSA and monthly reports completed in a timely fashion
Documentation Compliance: Will competently complete required clinical documentation such as EON, CANSA, monthly reports and staffing records to agency and referral source expectations
Be a team player demonstrating professionalism, cooperation and collaboration: By showing a willingness to accept supervision, completing required trainings and seeking out new development opportunities, attending required meetings and promoting open communication and ethical practice.
Quality of Care: By putting a focus on client centered treatment that is flexible to client’s emergent needs during bi-weekly supervision with Team Leaders

Experience and Education Qualifications:
Bachelor’s in Social Work, Psychology, Sociology or related Health and Human Services field

Employment Requirements:
Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.
Completion of New Hire Orientation at the beginning of employment.
All training requirements including Relias at the beginning of employment and annually thereafter.
Current driver’s license, acceptable driving record and current auto insurance.
Physical Requirements:
ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up the 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
