ESSENTIAL FUNCTIONS
• Collects data base information during the intake process. Provides appropriate documentation of referral per agency policies. Obtains initial insurance authorization and assists case managers in tracking/obtaining additional authorization as needed. Coordinate admission process with Nursing Staff.
• Communicates with and educates referral sources regarding services via written and telephone contact.
• Communicates appropriate information to team members regarding new admissions and referrals, including client needs such as a plan of care, supplies, and equipment. Serves as a liaison between referral sources, interdisciplinary teams.
• Investigates, verifies, negotiates and documents coverage from all payor sources.
• Advocates on behalf of client to ensure receipt of entitled benefits.
• Tracks all referrals/admissions on weekly referral report.
• Effectively assists the client, physician and/or discharging facility to establish an effective home care/hospice plan prior to the client’s transfer to home setting.
• Updates referral sources on status of clients currently receiving service or recently discharged and the client’s response to services received.
• Maintains clinical competency by assuming responsibility for acquiring knowledge and experience to meet learning needs.
• Runs HIQH check on every admission. Reviews monthly HIQH/insurance verifications on a monthly basis.
