Full Life Care - Case Manager (Beacon Hill)

If so, look no further Full Life Care, a Transforming Age Affiliate, is looking for Case Managers to provide direct service to our participants in need of skilled services in a community health setting. 

 

Our Case Managers


Determines clients’ situations and needs by establishing rapport with clients and caregivers; establishing service goals and plans; coordinating interdisciplinary assessments with team members. Address client personal care needs, providing assistance as needed. Assists in kitchen prep for client meals and clean up. Maintains continuity of care by communicating changes in client condition to team members; informing caregivers and physicians; documenting changes and service plan events; monitoring progress toward service goals and adjusting services as required. Promotes client independence, education and recreation by developing and facilitating activities according to clients’ abilities and interests; encouraging interaction and participation. Develops a safe, therapeutic group environment by establishing and keeping group rules; monitoring and encouraging appropriate participation; intervening as necessary. Maintains a safe and healthy environment by adhering to infection control protocols; addressing potential safety and health hazards; following emergency policy and procedures; and fulfilling education and certification requirements. Establishes a compassionate and respectful environment by practicing good communication skills; keeping client information confidential; conducting self in professional manner. Maintains required documentation to meet WAC guidelines and/or Full Life policy. Recording clients progress including individual interactions, provider communication, caregiver conversations and multidisciplinary meetings where appropriate. Develops and implements client care plans on a quarterly basis to ensure effective treatment plans. Maintains continuity of care by communicating changes in client condition to team members; informing caregivers and physicians; documenting changes and service plan events; monitoring progress toward service goals and adjusting services as required.
Completes projects and tasks as assigned by supervisor. Advises clients and caregivers by suggesting and exploring options; providing feedback and guidance. Obtains assistance for clients by coordinating care while at Full Life Care; arranging for appointments; making referrals to other services.


What you will need to succeed! 


Bachelor’s degree in human services or related field or high school diploma and two or more years experience in related field. Recommended that applicant have at least one year experience working with adults in a health care or social service setting (not required); Case Management experience. CPR certification: training will be provided on site Must complete the HIV/AIDS information course Employees are required to receive an annual influenza vaccine unless granted a medical or religious exemption. Required food handler’s permit
