Full-time 1st Shift Specialist, Community Health Care - H&W Non-Store Operations
Position Summary...

What you'll do...

We are hiring a Full-time 1st Shift Specialist Community Healthcare working Monday through Friday from 9am-6pm for our Health & Wellness facility in Orlando, FL. This position is responsible for assisting in the operation of a department. An individual in this position will be expected to perform additional job-related responsibilities and duties throughout the facility as assigned and/or as necessary.

Essential Functions An individual must be able to successfully perform the essential functions of this position with or without a reasonable accommodation.

Demonstrates up-to-date expertise and applies this to the development, execution, and improvement of action plans by providing expert advice and guidance to others in the application of information and best practices; supporting and aligning efforts to meet customer and business needs; and building commitment for perspectives and rationales.

Provides health resources and programs to customers by meeting customers and conducting needs assessments; assisting customers with the development of health management plans and goals; connecting customers with community resources (for example, senior centers, homeless shelters, food pantries); helping customers complete applications for benefits (for example, insurance, SNAP food benefits); assisting customers with the organization of personal matters (for example, financial and health records, appointment scheduling, prescriptions refills); and utilizing electronic databases to document and maintain records of customer encounters and interactions.

Provides and supports the implementation of business solutions by building relationships and partnerships with key stakeholders; identifying business needs; determining and carrying out necessary processes and practices; monitoring progress and results; recognizing and capitalizing on improvement opportunities; and adapting to competing demands, organizational changes, and new responsibilities.

Working with customers to meet their health goals by contacting customers in person, by phone, or in writing to ensure they have completed required or recommended actions; conducting customer interviews and providing coaching to identify and troubleshoot barriers and connect to additional resources as needed; providing feedback to health service providers to improve service accessibility or acceptability; working with state and federal public health programs to improve resources, services, and programs.

Models compliance with company policies and procedures and supports company standards of ethics and integrity by incorporating these into the development and implementation of business plans; using the Open Door Policy; and demonstrating and assisting others with how to apply these in executing business processes and practices.

Collaborates with the community health worker team; creates and maintains a repository of community resources by using Microsoft Office (for creation), SharePoint (for housing/organizing), and Zoom teleconferencing, using other technology platforms for communication when appropriate; coordinates and supports community events in the Walmart Health Center and in the local community; performing data entry utilizing a computer and various software.

Increases customer awareness and utilization of resources, services, and programs by conducting outreach for medical personnel or health organizations to implement community health programs; expanding knowledge and understanding of community health resources, services, and programs; working with other community healthcare workers to create a directory of community resources; conducting group presentations and health screenings within the Walmart Health Center and community settings; collaborating with the behavioral health and physical health care teams regarding customer problems and needs; and attending community meetings, health fairs, and community advisory committee meetings to understand community health issues and build relationships with community healthcare members.

Provides and supports the implementation of business solutions by building relationships and partnerships with key stakeholders; identifying business needs; determining and carrying out necessary processes and practices; monitoring progress and results; recognizing and capitalizing on improvement opportunities; and adapting to competing demands, organizational changes, and new responsibilities. Minimum Qualifications

2 years’ experience in community outreach, customer service, social service, or related field.

1 year's experience working autonomously with minimal supervision.

1 years’ experience using intermediate functionality of Microsoft Office Suite.

The hourly wage range for this position is $25.60-$38.40*
