As a Community Occupational Therapist you be responsible for the provision of OT services, to Ontario Health atHome HNHB eRehab patients in order to maximize the patient’s function, independence, and safety in their respective environment. Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient-centred care – and be supported by our collaborative team that includes over 9,000 regulated health care and other professionals. What will you do?

Respond to patient referrals from the Care Coordinator and follows Ontario Health atHome procedures to properly administer eRehab services to patients in a Directing Therapist Role Seeks informed consent from patients before commencing assessment and treatment planAssesses the factors influencing the patient’s ability to function in their environment, using a variety of standardized and non-standardized assessment tools, to determine patient treatment needsDetermines patient centered therapy goals and provides treatment program to increase occupational performance, independence, and safetyUtilizes Sensory Tech Platform (and/or other organizational platforms e.g. OTN) to provide clinical support to Rehab Assistants who work with the patient directly to implement treatment plan Teach and assign specific aspects of patient care to a person deemed competent and capable to carry them out and accepts responsibility for these delegated tasks according to standards of the College of Occupational Therapists of Ontario (COTO)Reassesses patient progress on an ongoing basis, and adjusts treatment program when needed, to ensure that therapeutic goals are achievedManage service delivery in conjunction with the Most Responsible Physician, Care Coordinator, patients, their family members/caregivers/substitute decision-makers (SDMs), and other service providers, via virtual models of care (e.g. video, telephone, secure messaging), in person visiting and written correspondenceDischarge patients when appropriate and refers patients to other internal or external services if necessaryAdvocate for patients as warranted to identify lack of resources/services for patients and to participate in developing alternative resources within the communityUses sound professional judgment and acts in accordance with all Ontario Health atHome policies and procedures and standards of practice set forth by COTOMaintain professional competence and knowledge of current practices by participating in professional development and ongoing researchParticipate in program development and quality improvement initiatives to improve patient careWork through Sensory Tech Platform for eRehab services and may require occasional travel throughout the HNHB region as require for patient and community engagementContribute to a safe and healthy environment by following safe work procedures, reporting injuries, illnesses, and unsafe working conditionsDocuments and charts assessment findings, goals, treatment plans, and interventions on behalf of the patient in accordance with Ontario Health atHome policies and procedures, Sensory Tech requirements, and as per guidelines established by COTOProtects patient’s privacy and confidentiality in accordance with legislation, Ontario Health atHome policies, and standards established by COTOMaintains accurate statistics on direct and indirect patient care, submits records of weekly visits and mileage reports, and performs other administrative duties as required, in accordance with Ontario Health atHome policies and proceduresAbility to utilize electronic databases, including electronic patient records, and documentation platforms.

What you must have?

Graduate of an accredited degree program in Occupational Therapy (OT) Current Registration in good standing with the College of Occupational Therapists of Ontario requiredAt least two years of recent clinical work experience as a directing occupational therapist requiredWe have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date
