Position Purpose:

Provides individualized nursing care/assessments at the home/residence of the client diagnosed with acute, chronic and complex conditions.

What you will Do:

Analyzes and differentiates clinical data to appropriately diagnose nursing problems; and identify nursing diagnosis.Provides individualized care to clients with complex care needs.Demonstrates the ability to utilize critical thinking skills in the home environment and identify situations requiring physician notification or emergent care.Demonstrates the ability to care for clients with a variety of diagnosis’ including but not limited to orthopedic, surgical, medical, cardiac, and wound/ostomy.Provides health education to patients/families and populations focused on achieving lower costs, engaged patients and reduced hospitalizations and ER/Urgent visit usage.Delivers nursing care for the clients in their homes, utilizing appropriate available resources.

Hours/Schedule:

Monday through Friday 8-4:30. Flexible schedule.

Minimum Qualifications:

Current state LPN license.Must provide proof of Basic Life Support (BLS Provider-American Heart Association or Professional Rescuer-American Red Cross) prior to employment or within 30 days of hire and maintains current status.Must provide proof of Mandatory Reporting for Child and Dependent Adult Abuse training prior to employment or within 6 months of hire and maintain current status per State of Iowa guidelinesMotor Vehicle: Personal Vehicle. If required to operate personal vehicle, will need to have proof of and maintain valid driver's license and adequate insurance coverage on file with Human Resources
