Overview A Health Coach partners with patients to assess their lifestyles, address their health concerns, and set short- and long-term goals as ordered by the patient’s healthcare provider. Health coaches support patients in setting personalized health goals that promote a patient-centered vision of health and well-being. The Health Coach will help patients in finding new ways to inspire healthier habits. The position will work to promote wellness and helping individuals realize their optimal well-being. The Health Coach will be responsible for developing a health coaching relationship with patients and team members and assisting them through the process of actively working towards better health by providing support, encouragement, and education. Recommended Dietary Allowance and scientific principles applicable in a variety of disease conditions. Job Details Duties and Responsibilities: Provides necessary coaching to support behavior change, on either individual or group basis. Utilizes the coaching model process to help patients clarify vision and values, assess readiness to change, determine focus and set goals, prepare for action, and maintain behavior change. Incorporates skills such as active listening and presence, motivational interviewing, reflection, and goal-setting to assist patients in making change. Establishes patient-centered relationship that recognizes that the patient is the expert in navigating their own lives, and provides the structure to serve as a facilitative partner. Contacts and performs initial interviews with individuals who are eligible for health coaching programs. Identify and provide appropriate resources to patients where needed. Supports the operational aspects of the Integrative Medicine Program in order to promote patient satisfaction. Develops and conducts seminars patients or employees who are interested in learning more about health and wellness. Creates and distributes health education materials such, as brochures, to patients. Coordinates with patients other healthcare providers in order to provide a well-rounded experience and optimize patient outcomes. Advises employees in food preparation and service regarding clinical nutrition Salary Range USD $37.00 - USD $54.00 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies, and experience and position location. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
