Vancouver Coastal Health is seeking a dedicated and compassionate provider (GP/NP) to join an innovative outreach role which will support clients in supported housing. This role is critical in providing episodic care to diverse populations across our community, focusing on urgent health needs with a strong emphasis on mental health, substance use, and systems coordination.
As a General Practitioner with Vancouver Coastal Health, you will:
In this role you will act as the MRP for patients in the following housing models:
Victory House (hybrid):
Longitudinal primary care for 45 total clients (34 of them are attached to the Strathcona Mental Health & Substance Use team with chronic, severe mental health issues)To round in person 1 week a month with virtual support otherwise from 9-5 Monday-FridayTime: 4 hours a weekComplex Care Housing (hybrid):
Episodic/bridging primary care for vulnerable clients in complex care housing sites across VancouverThese sites have nurses embedded who would request consultation for episodic care issues as well any primary care needs that come upThis work could be done virtually or in person with an outreach visitTime: 8 hours per week QualificationsEducation, Licensing, & Experience:
Current College of Physicians of BC (CPSBC)- ProvisionalCanadian College of Family Practitioners (CCFP) - EligibleCanadian Medical Protective Association (CMPA)- MandatoryKnowledge & Abilities:
Provide episodic care to patients who will be initially assessed by their respective housing site’s nurseThis could be done virtually or in-personLiaise with nursing staff create an episodic care plan for a patient and determine which services they will be bridged/connected toCoordinating with the other physicians/NPs and/or other VCH outreach teams as needed to ensure continuity of care
This episodic care could require:
Skin and soft tissue infection (SSTI) managementBridging scripts for opiate agonist managementAntipsychotic titration for mental health decompensationVirtual certification with a Form 4 (on rare occasions)Care coordination, as well as liaison with specialistsRequirements:
MD degree with a current license to practice in British Columbia.Experience in primary care, emergency medicine, psychiatry, or related fields.Proficiency in virtual care platforms and electronic medical records.Excellent communication and interpersonal skills.Commitment to patient-centered care and harm reduction principlesYou will need to use Profile EMR, PARIS as well as Cerner for this role, and will be compensated for any training required.
