Hospice of Michigan is looking for a Grief Support Services Manager passionate to provide high quality patient care to join our team!
The Grief Support Services Manager is responsible for designing, developing and implementing grief support services and educational programs for bereaved individuals including Hospice of Michigan (HOM) families and community members. The Grief Support Services Manager will evaluate all outcomes in support of continuous improvement for community outreach activities.
+ Develops bereavement plan of care, reflecting family/caregiver needs and frequency of services and provides all services in accordance with the individualized plan of care.
+ Participates in regular Interdisciplinary (IDT) meetings.
+ Practices the client-centered model which creates an environment conducive for healing to occur including the provision of grief companion visits.
+ Facilitates memorial services, in collaboration with patient care teams, for HOM families as well as the community.
Qualifications:
+ Master's degree required. Preference will be given to Social Workers with State of Michigan LMSW license. Formal education and experience in guidance and counseling is required; various combinations of education, training and experience may be considered.
+ Must demonstrate knowledge of bereavement/spiritual concepts and have the ability to articulate these concepts to a personal death experience.
+ Must have minimum of one (1) year grief support work experience within a recognized agency or health care organization.
+ Must demonstrate knowledge of behavioral and psychological stages of adults, children and teens, in order to facilitate the grief process with all persons.
+ Demonstrated ability to organize and conduct grief support groups.
+ Must be able to cope with family/caregiver emotional stress, cultural diversity and be non-judgmental of individual life-styles.
+ Ability to use technology (hardware and software) HOM requires clinicians to use in their practice such as computer, cell phone, printer, pager, fax and MISYS software (data entry for documentation, uploading and downloading) required.
+ Must be dedicated to the philosophy and purpose of hospice and palliative care.
+ Must possess sound judgment; prioritization and follow-through skills; attention to details; dependability; tact; and ability to maintain confidential information.
