Habilitation Aide
Job Brief

Assists Day Treatment staff in the provision of active treatment services including the development and implementation of individual treatment plans for participants in day treatment and day habilitation settings.

Shift Worked: 8:00AM-4:00PM Weekly Hours: 37.5 FLSA Status: Non Exempt Pay range: $17.74-17.74

POSITION DUTIES:

Attends participant annual, semi-annual, and special conferences as needed. Assists habilitation counselors and clinical team members in the evaluation of participants and participates in the development of the participant’s individual treatment plan. Implements participant’s individual treatment plans by following prescribed methodologies. Maintains records and charts for each participant by entering required information, such as: goal/data sheets, behavioral data sheets, inclusion forms, body checks, and residential program’s communication book. Assists participants with activities of daily living including toileting, showering, grooming, oral hygiene, meal programs and dressing as required. Monitors participant’s physical well being and reports any changes in condition and/or health care concerns to nursing staff. Accompanies participants to and from buses/vans and ensures their safety when getting on and off vehicles. Monitors participant’s lunches/snacks and reports any dietary concerns to nursing staff. Accompanies participants to emergency room, doctor appointments as assigned. Participates in special program/projects, inclusionary and volunteer opportunities (i.e., recreation, vocational, community trips, etc.). Attends mandatory in-services and specialized training programs as assigned. Prepares and sets up classroom for day’s activities prior to participant’s arrival. Maintains cleanliness and safety of assigned work area. Performs all other duties as assigned.

ESSENTIAL PHYSICAL REQUIREMENTS:

The ability to coordinate the movement of the arms, legs, and torso together in activities where the whole body is in motion, including the ability to exert one’s self physically over long periods of time such as standing for two (2) plus hours at a time. The ability to safely lift individuals to/from various surfaces. The ability to safely perform a two (2) person transfer. Perform a stand pivot transfer with weight bearing individuals. The ability to lift and reach common objects such as laundry and nonperishable food items. The ability to push/pull, turn and maneuver equipment including occupied wheelchairs. The ability to bend, stretch, rotate, or reach out with the body, arms, and/or legs to ensure the safety of individuals in all situations.

QUALIFICATIONS:

Completion of OMRDD Approved Medication Administration Course. Completion of Cerebral Palsy orientation training program. Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
