Habilitation Coordinator (All Boroughs)
Job Brief

Responsible for the supervision of Habilitation Counselors & Habilitation Aides. Designs, implements & monitors individual program plans for individuals served. Encourages, implements & facilitates independence, inclusion, individuality & productivity.

Shift Worked: 8:00AM - 4:00PM [Monday - Friday] Weekly Hours: 37.5 FLSA Status: Non Exempt Pay range: $27.18 -$28.21

Our dedicated staff provides a wide range of direct services that address the individual needs of people with developmental disabilities, including those with the most severe disabilities.

In our Day Programs, CP Unlimited offers a comprehensive, intensive array of services individualized to meet each of the people that participate in our programs unique needs to learn skills for community living to the extent he or she is capable. Staff and families work together to provide programs that include opportunities for the people to participate in integrated, community based activities. 

CP Unlimited is currently seeking a full-time Day Habilitation Coordinator to join our team of professionals in our Bronx West Farms Day Center to supervise Counselors and Aides; implement and monitor program plans for the people who attend our center; prepare notes, files and attend participants’ conferences; and encourage, implement and facilitate independence, inclusion, individuality and productivity. 

Requirements: • Bachelor’s Degree in education or a related field • 2 years’ experience working in a program for people with developmental disabilities • QIDP status • Supervisory experience preferred.
