UBHC’s 1,060 experienced behavioral health professionals and support staff are dedicated to treatment, prevention, and education. UBHC, one of the largest providers of behavioral health care in the country, has a budget of $260 million and has 15 sites throughout New Jersey. Services are readily accessible and include: inpatient, outpatient, partial hospitalization, screening, crisis stabilization, family/caregiver support, community outreach and case management, supportive housing, supported employment, prevention and consultation, employee assistance programs, and a licensed therapeutic school from preschool through high school.
Specialty services include the New Jersey suicide prevention helpline and peer help lines for police, veterans, active military, teachers, mothers of special needs children and child protective service workers. In FY2016, UBHC treated 16,199 consumers, had 24,502 admissions, and touched the lives of 19,441 individual callers through peer support. In addition, UBHC is the primary mental health training resource for the New Jersey departments of Human Services, Children and Families, and Corrections, delivering 16,000 trainings each year. Posting Summary Rutgers The State University of New Jersey is seeking a Habilitation Counselor in the Department of Outpatient Treatment – Certified Community Behavioral Health Clinic (CCBHC) Case Management of Rutgers University Behavioral Healthcare.
The Habilitation Counselor provides community-based case management services for individuals served to include assessment, treatment/recovery planning, linkage and coordination of care.
This position will focus on individuals within the Just In Time scheduling process within the CCBHC programs.
Among the key duties of this position are the following: Supports individuals to gain access to needed medical, social, education, mental health, addiction, co-occurring and other services. Links individuals served to needed services including: benefits, medical, dental, mental health, co-occurring services, self-help, employment, legal and housing. Assists with scheduling appointments with the individual served and when necessary accompanies the individual served to appointments. Demonstrates the ability to engage and establish a working relationship with, and provide services to individuals who are diagnosed with a serious and persistent mental illness and co-occurring disorders who present engagement challenges. Utilizes Motivational Interviewing skills to establish a working relationship with individuals served. Provides services that are Person Centered that assure that the person’s expressed needs and strengths drives the service Plan and service delivery. Collaborates effectively with members of the department, and with external departments, staff, referral sources, other service providers and social support networks to provide services and rehabilitative care for consumers and families. Maintains appropriate clinical records according to the time frames and quality guidelines established by UBHC policy and applicable accrediting organization standards. Understands and adheres to Rutgers’ compliance standards as they appear in RBHS’s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy. Keeps abreast of all pertinent federal, state and Rutgers’ regulations, laws and policies as they presently exist and as they change or are modified.
