**JOB OVERVIEW**
The Health and Wellness Coordinator in an independent living (IL) community is responsible overseeing and coordinating nursing and clinical activities within the Wellness clinic, ensuring a welcoming and responsive environment that prioritizes the well-being and safety of all residents. This role focuses on supporting clinical staff, coordinating resident healthcare needs, providing preventative and wellness services, and offering health education and support. The Health and Wellness Coordinator works closely with residents, families, and healthcare providers to ensure optimal care and quality of life within the community.
**RESPONSIBILITIES & QUALIFICATIONS**
**Responsibilities:**

The unique responsibilities for this role include but are not limited to the essential functions listed as follows:
+ Manage the overall operations of the Wellness clinic.
+ Monitor the health and safety of residents in Independent Living through collaboration with third party services and the management team.
+ Schedule appointments with residents and make referrals to specialists as ordered by community physician.
+ Collaborate with community leadership team for inter-community referrals.
+ Partner with Director of Sales (DOS) for external business development and present Independent Living service offerings.
+ Participate in community events and meetings as directed by Executive Director.
+ Order labs and X-rays as ordered by community physician.
+ Triage residents and coordinate with community physician/or resident’s primary physician, follow-up (Transfer to Hospital, schedule / same day or future appointment) with physician.
+ Coordinate with community physician to complete assessments for potential residents to determine eligibility for Independent Living.
+ Coordinate with external pharmacy for vaccination clinics.
+ Participate in resident discharge planning as needed.
+ Coordinate services with external providers for residents including audiologist, podiatrist, dentist, and rehabilitation etc.
+ Manage the clinic staff including Medication Technician and Licensed Practical Nurse (LPN).
+ Maintain compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.
+ Perform other duties as assigned.
**Qualifications:**
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed throughout this job description are representative of the knowledge, skills, and abilities required.
+ Registered Nurse (RN) with an active state license; Bachelor’s degree in Nursing (BSN) preferred
+ Minimum of 2 years of nursing managerial or supervisory experience, ideally in a senior living or community health setting
+ Basic Life Support (BLS) certification; additional certifications in geriatric or community health nursing are desirable
+ Familiarity with electronic health record (EHR) systems
+ Proficient in use of Microsoft skills
