Summary :The Health Coach facilitates health promotion programs to assist team members and/or spouses in identifying their health risks. Supports team members and/or spouses to achieve personal wellness goals and helps reduce barriers to a healthy lifestyle. Serves as a catalyst for change, supporting a culture of health and well-being in the work environment and community.Job Description:KEY ACCOUNTABILITIES:
Reviews health risk assessment results to help participants understand lifestyle-related health risks, assess readiness for change, and use motivational skills to set health related goals and develop and implement effective strategies to reach those goals.
Provides health coaching in individual and group settings.
Documents goals, clinical data, and behavioral outcomes appropriately in the electronic medical record.
Collaborates with leaders, wellness team, departments, and external clients to promote and support wellness initiatives.
Plans and/or assists with health-related demonstrations, programs, displays, and materials to support individuals in their wellness journey, and promotes a culture of health and well-being.
Implements, promotes, and leads health, wellness, and chronic disease programs that emphasize the importance of healthy lifestyles such as weight management, nutrition, smoking/nicotine cessation, mindfulness-based stress reduction, fitness evaluations, activity, and other programs.
Collaborates on the development of programs that promote healthy lifestyles that include step-by-step implementation plans, program details, resources needed, goals for success, and all tools needed to successfully complete the program.
Contributes to and maintains the ThedaCare “health coach toolkit” to stay current on new evidence-based research and innovative techniques to promote health, wellness, and chronic disease prevention and management.
QUALIFICATIONS:
Bachelor’s degree from an accredited institution in health education, health promotion, exercise science, nutrition, psychology, or other health-related subject
American Heart Association Basic Life Support (BLS)
One year of experience developing, monitoring, measuring, communicating, and reporting on health-related topics
PHYSICAL DEMANDS:
Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance
Job classification is not exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
WORK ENVIRONMENT:
Climate controlled office setting with daily movement throughout the campus
Interaction with department members and other healthcare providers
Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock (onsite health coach)
Occasional high noise level in work environment (onsite health coach)
Possible exposure to communicable diseases, hazardous materials, and pharmacological agents
Occasional contact with aggressive and or combative patients
Position requires compliance with department specific competencies.
