The primary function of the Care Transitions Health Coach is to promote health care activation for individuals involved in health services along the continuum of care. Duties are geared toward providing interventions that decrease care fragmentation, improve care outcomes, enhance individual patient activation and decrease unnecessary hospital re-admissions. Responsibilities are focused on patient-centered activities that enhance care coordination with other community providers. Goals are centered on the Triple Aim: Improving the patient experience, improving quality of care and decreasing unnecessary health care expenditures.

Required Qualifications:
Bachelor's Degree Related field. upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information. 1 year In Community outreach.Preferred Qualifications:
