**Description** The primary function of the Care Transitions Health Coach is to promote health care activation for individuals involved in health services along the continuum of care. Duties are geared toward providing interventions that decrease care fragmentation, improve care outcomes, enhance individual patient activation and decrease unnecessary hospital re-admissions. Responsibilities are focused on patient-centered activities that enhance care coordination with other community providers. Goals are centered on the Triple Aim: Improving the patient experience, improving quality of care and decreasing unnecessary health care expenditures.
