**Position Summary**
**This Health Coach Consultant RN role is telework and must live in North Carolina.**
Must have a compact license in NC and acute Medical-Surgical Nursing and chronic condition education experience with an adult population. It requires proficiency in delivering telephonic 1:1 coaching.
**The position is full time, 40 hours/week,**
**Work Schedule:**
**Monday-Friday: 9:00 am – 5:30 pm EST; 3 set days a week along with 2 set evening shifts per week from 12:30-9:00 pm EST**
**Weekends: rotating late Friday until 9 pm EST (typically every 4-6 weeks) and rotating Saturdays are required, per business needs (typically every 4-6 weeks)**
The Health Coach Consultant: Chronic Condition Nurse utilizes a collaborative process of assessment, planning, implementation, and evaluation, to engage, educate, and promote and influence member's decisions related to achieving and maintaining optimal health status. You will assess members through the use of clinical tools and information/data review to conduct comprehensive evaluation of needs and benefit plan eligibility for available integrated internal and external programs/services. You will also utilize assessment techniques to determine member's level of health literacy, technology capabilities, and/or readiness to change.
**Enhancement of Medical Appropriateness & Quality of Care:**
Application and/or interpretation of applicable criteria and guidelines, health/wellness management plans, policies, procedures, regulatory standards while assessing benefits and/or member's needs to enable appropriate utilization of services and/or administration and integration with available internal/external programs
Using holistic approach consults with supervisors, Medical Directors and/or others to overcome barriers to meeting goals and objectives
Identifies and escalates quality of care issues through established channels.
Utilizes influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health
Provides coaching, information and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices
Interprets and utilizes clinical guidelines/criteria to positively impact members health Provides up-to-date healthcare information to help facilitate the member’s understanding of their health status
Helps member actively and knowledgably participate with their provider in healthcare decision-making
**Monitoring, Evaluation and Documentation of Care:**
Develops and monitors established plans of care, in collaboration with the member and/or attending physician, to meet the member's goals
Utilizes internal policy and procedure in compliance with regulatory and accreditation guidelines
**Required Qualifications**
Must hold an unrestricted RN compact license in state of NC and have the ability to be licensed in all non-compact states.
3+ years of recent clinical experience in Adult Medical Surgical floor nursing
3+ years of recent clinical experience providing health education about chronic health conditions to adults
3+ years of experience with documenting clinical notes in an electronic medical records (EMR/EHR) system
3+ years of proficiency with Microsoft Office application: Word, Outlook, Excel, Teams and Power Point experience
2 + yeas of experience to delivering health education coaching via telephone 1:1
**Preferred Qualifications**
Previous telephonic chronic condition coaching
Virtual group coaching experience via webcam
Managed care experience
Experienced in Epic EMR data base functionality and documentation
Excellent communication skills and must be comfortable with public speaking
Ability to work independently as well as part of a group
**Education**
Associate’s or Bachelor’s degree in Nursing
**Pay Range**
The typical pay range for this role is:
$54,095.60 - $116,760.80
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
