SUMMARY (BASIC PURPOSE OF THE JOB)Works collaboratively with physicians, staff and other health care professionals to provide a medical home and coordinate care across the health care continuum for all patients within the hospital setting and to support the transition back to the community. Promotes patient-centric care and actively participates in multidisciplinary patient-centered team meetings. Follows all established protocols, policies, procedures and standardized work flows. Assists in managing data so that patients are effectively treated.MINIMUM REQUIREMENTSEducation: High School diploma or equivalent, Associate's degree preferred. Certified pharmacy tech preferred
Experience: Experience in a health care related field preferred.
Other Credentials: AHA BLS. Non-Licensed must obtain before end of orientation period
Knowledge and Skills: Excellent interpersonal, verbal and written communication skills. Strong organizational and time management skills. Demonstrates motivational and team building skills.
Special Training: Proficient with Microsoft Excel and Word. Basic and mathematical skills. Basic understanding of pharmacology and medical terminology. Second language a plus.
Mental, Behavioral and Emotional Abilities: Demonstrated ability to influence others while motivating them to change; ability to multitask and prioritize while working in a high-volume environment. Ability to work independently, as well as within a team.

.ESSENTIAL FUNCTIONSWorks independently to obtain and review pertinent medical history, current diagnosis, and pharmaceutical data for Population Health identified patients.Provides telephonic coaching and information to Population Health patients, managing various chronic health conditions.Communicates with Population Care Coordinator, physicians, and other health professionals to provide care coordination that facilitates the efficient use of health care resources.Reviews, clarifies, documents, and verifies home medication list and allergies of all Population Health patients. Clarifies and records the nature of the allergic reaction when necessary.Effectively coaches, evaluates and graduates patients from care in conjunction with the care management team.Assists in the transition of care across the continuum. Uses evidence-based coaching techniques to help members achieve healthy lifestyle behavior challenges, align lifestyle behaviors with their personal wellness goals, and design a realistic action plan in support of a healthier lifestyle.Provides exceptional service and/or care to all patients, customers and team members. Engages, educates, and promotes/influences members in decisions related to achieving and maintaining optimal health status.
